James Maddison Injury Blow Deepens Tottenham Injury Concerns
Tottenham have been dealt another early setback, with The Athletic reporting that James Maddison has fractured his shoulder and could be sidelined for up to two weeks. For a squad already carrying too many injury burdens, it is another frustrating update at the worst possible time.
The problem came after a fall in Spurs’ Premier League-opening 3-0 defeat to Brentford. An MRI scan then revealed a slight break, although there is some encouragement around the severity. Roberto De Zerbi confirmed on Tuesday that Maddison had suffered a “small injury” to his shoulder during his substitute outing, while there had already been a “small problem” in the summer that limited his pre-season involvement.
Maddison injury latest worries Spurs
On its own, a two-week absence may sound manageable. In the context of Maddison’s recent history, it feels far more significant. The 29-year-old missed almost all of the 2025-26 campaign with a ruptured ACL and only returned in May, first appearing off the bench in the 1-1 draw against Leeds before two more substitute outings.
That came after a miserable sequence in which Maddison suffered a partial ACL tear against Bodo/Glimt in May 2025, then sustained a full rupture when trying to return in pre-season against Newcastle United. It meant 12 months out of competitive football, another cruel interruption for a player who had already lost around two months of his debut Spurs season to an ankle injury.
Tottenham injuries continue to mount
This is where the bigger concern lies for Tottenham. Maddison’s latest problem arrives with De Zerbi already navigating a stretched squad. Xavi Simons, Wilson Odobert and Dejan Kulusevski are all out with ACL injuries, while Mohammed Kudus is only just back in training after a quad issue. Micky van de Ven and Pape Matar Sarr missed the Brentford game, and Pedro Porro was absent after a shortened pre-season following Spain’s World Cup success.

Spurs have made a habit of looking patched together far too often in recent seasons. Maddison, signed from Leicester City in June 2023 for over £40million ($54.6m), has made 79 appearances for the club, but the lingering theme has been stop-start availability rather than sustained rhythm.
De Zerbi faces early Spurs test
De Zerbi will hope this shoulder issue truly remains minor. Even so, Tottenham’s season already feels defined by who is unavailable rather than who is ready to drive them forward. When one of the squad’s most creative midfielders suffers another knock, it naturally raises fresh questions about resilience, depth and whether Spurs can finally break this damaging cycle.

From a Spurs supporter’s perspective, this is exhausting. Every time there is a hint of momentum, another fitness update lands and drags the mood straight back down. Hearing that Maddison has a “small injury” offers some relief, but fans have heard reassuring language before and then watched key players disappear for weeks or months.
Maddison is one of the few in this squad who can genuinely slow a game down, pick a pass and give Tottenham some personality in the final third. After everything he has gone through with the ACL nightmare, you just want to see him catch a break and build some rhythm. Instead, this report brings back the same old fear that his Spurs career will be remembered for absences as much as influence.
The wider issue is even more damaging. Tottenham cannot keep entering important stretches with half the core squad missing. It affects confidence, balance and the whole identity of the side. Supporters can accept bad luck now and then, but this has gone beyond that, it feels like a pattern that keeps sabotaging every attempt to build something coherent.
There is still time for this to settle, and if Maddison only misses a short spell then Spurs should be grateful. Even so, the opening weeks already feel tense. After a 3-0 defeat to Brentford, the last thing any gutted Spurs fan wanted was another injury headline centred on one of the club’s most important players.
